Outpatient Services & Medication Assisted Treatment

 

NCTR COUNSELING CENTER

The NCTR Counseling Center offers tiered programming for adults and adolescents, from highly intensive clinical intervention to self-management.

  • Adult Partial Hospital Program (PHP):

  • PHP is the most intensive level of outpatient care consisting of 6 hours/5 days for up to 4-6 weeks.

  • Adult Intensive Outpatient Program (IOP):

  • IOP patients typically participate three times a week for 3 hours each day, for up to 6-8 weeks. 

  • Adolescent Intensive Outpatient Program (IOP):

  • IOP patients typically participate three to five times a week for 2 hours each day, for up to 6-8 weeks. 

  • Adult and Adolescent Outpatient Program (OP):

  • OP provides continuing care as patients step down from more intensive counseling. Patients typically begin by attending an average of two times/week and gradually progressing to attending 1 time per month over a 6-12 month period.

  • Adult and Adolescent Medication Assisted Treatment:

  • Medication-assisted treatment (MAT)  combines the use of medication with counseling and behavioral therapies to assist withdrawal and on-going recovery management in an office based setting.

Leadership

for NCTR Counseling Center

Jennifer Walden

Jennifer Walden, LPC, LSATP, CSAC

Chief Clinical Officer

  • As Deputy Chief Clinical Officer, Jennifer Walden oversees NCTR’s Assessments and Admissions Department as well as the agency’s Counseling Center outpatient programs.

    She previously worked for NCTR from 2004 to 2015, holding multiple clinical positions. After leaving NCTR, she worked as a therapist at Potomac Health Services, before rejoining NCTR.

    Jennifer is a licensed professional counselor and licensed substance abuse treatment practitioner and holds a Master of Science in education (community agency counseling) from Old Dominion University. She is also a Certified EMDR Therapist. “I am motivated by the changes I see in our patients every day -- I have the honor of watching our patients bravely confront an often-stigmatized illness.”

 ​​​​Heather El‑Zein, LPC

Director of Outpatient Services

  • Heather El-Zein is a Licensed Professional Counselor with a Master of Arts in Addiction Counseling from Liberty University. She began her career in the addiction treatment field in 2020, following a successful 25-year career in the hospitality industry, where she held multiple leadership roles with top-tier hotel companies.

    Heather transitioned into the behavioral health field through business development while completing her graduate studies. She completed her LPC residency providing both group and individual therapy with New Paradigm Recovery, where she specialized in Intensive Outpatient Program (IOP) services. She also served as Lead Therapist at Sandstone Care’s Detox and Residential Treatment Facility. Heather has extensive experience working with individuals experiencing co-occurring substance use and mental health disorders.

    In addition to her work in treatment settings, Heather maintains a private practice with Northern Virginia Counseling Services. She is deeply committed to creating a safe, supportive, and non-judgmental environment where clients feel empowered to be authentic and share their stories. Heather believes that trust and genuine connection are foundational to helping individuals develop greater awareness of the thoughts, emotions, and behaviors that impact their ability to function in healthy and meaningful ways.

    Heather is also passionate about community service. She helped establish Loudoun Serenity House, a nonprofit organization dedicated to sober living and ongoing peer support for individuals seeking accountability and sustained recovery. She remains actively involved in supporting recovery-focused initiatives throughout Loudoun County.

    In her personal life, Heather enjoys spending time with her partner, Blaine, her two grown sons, Ramzy and Gavin, and their dog, Kody.

Susan Spies, MS/MPH, FNP

Nurse Practitioner Manager

  • Susan Spies manages the Office Based Addiction Treatment (OBAT) program at NCTR’s Counseling Center, where she prescribes Medication Assisted Treatment (MAT) for patients suffering from opioid use disorder and alcohol use disorder. She also provides MAT education to patients and staff. She has 20 years of experience in nursing, including nine years in addiction medicine. Prior to her current position she held several positions at NCTR, including family nurse practitioner in the residential programs and director of nursing. Before joining NCTR, she worked for several years at Neighborhood Health, where she provided primary care as a family nurse practitioner. She derives great fulfillment and gratitude from “the patients that check in with me one, two, even three years later to let me know how they're doing and that they are still sober.” Susan holds a master’s in nursing and master’s in public health from Johns Hopkins University.

Mark Sakran, MD

Medical Consultant

  • Dr. Sakran was born and raised in Fairfax, Va. He received hismBachelors of Science in Biology from George Mason University and obtained his Doctorate in Medicine from The University of Saint Eustatius School of Medicine. He completed his General Adult Psychiatry residency at Hennepin-Regions Hospital in Minneapolis, Minnesota and his Child and Adolescent Psychiatry sub-specialty training at Georgetown.

    Dr. Sakran strives to thoroughly understand the needs of each patient. He approaches treatment by focusing not just on the individual, but rather by working closely with all those involved in the patients’ care to create a personal and effective treatment plan unique to each client. He has extensive training and experience in treating a wide array of disorders including Substance Use Disorder, ADHD, oppositional defiant disorder, OCD, Bipolar, anxiety and depressive disorders.  

     Dr. Sakran has worked with National Capital Treatment & Recovery in various roles since 2016. Originally working with our adolescent programs, he transitioned to focused on adult residential treatment and adult outpatient treatment. Dr. Sakran is now our Medical Director of our Office Based Addiction Treatment (OBAT) program, and our collaborating physician with Susan Spies, FNP. The OBAT program serves both adult and adolescent patients.

Kedest Gebreselassie

Nurse Practitioner Manager

  • Kedest Gebreselassie is a board-certified Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) and Family Nurse Practitioner (FNP-BC). She has devoted her career to improving the mental health and well-being of individuals across the lifespan. With more than 20 years of clinical experience, she brings deep expertise in the assessment, diagnosis, and treatment of a broad range of psychiatric conditions. 

    Kedest completed her undergraduate nursing studies at George Mason University, earned a Master of Science in Nursing with a specialization in Family Practice at Marymount University, and later obtained a post-graduate certificate in Psychiatric-Mental Health from Shenandoah University. Fluent in English, French, and Amharic, she began her mental health career in outpatient psychiatry, caring for diverse populations and coordinating closely with primary care to support whole-person health. 

    Over the years, Kedest has practiced primarily in outpatient behavioral health settings, providing individual and group interventions and comprehensive medication management for depression, anxiety, bipolar disorder, schizophrenia, PTSD, and substance use disorders. Her work includes experience with Medication-Assisted Treatment (MAT) and collaborative care models that span intensive outpatient through community-based recovery supports
